Typical use: Suicide, self-harm, violence risk, post-crisis discharge.

This is a clinical safety plan (Stanley–Brown structure). It is not the housing “bed tonight” sheet on EmergencyHousing.ca and it is not a no-suicide contract. Complete it with the person. If risk is imminent, stay with them and use 9-1-1 / Form 1 pathways — do not treat this form as a substitute for duty of care.
