Worker-to-worker transfer: what was tried, what is booked, what is still open.

Typical use: ED to community, hospital to outreach, treatment to primary care.

Warm handoff means a real conversation between workers, preferably with the person present. Faxing a summary into a void is a cold drop. Share only what is needed. Confirm identity on the phone. This page is not an electronic health record.
